Take these steps to get it all done!
2026/2027 registration will open up May 1st, 2026 (with Cheer opening March 1st).
Get registered here:
Sports Physical:
Visit your medical doctor or an urgent care to get an annual sports physical. Your sports physical may NOT be filled out by a Chiropractor.
- Page 1 is to be filled out by athlete/guardian
- Page 2 is to be completed by Physician/NP
Turn in the completed physical to the PHS front office or upload directly to your Home Campus registration. DO NOT give your sports physical to your coach.
Your Part is Finished!
Your student will be cleared by the athletic office once they have completed the above steps and a GPA of 2.0 or higher is verified during grade checks. *Incoming Freshman will not have the GPA requirement to be cleared for the fall semester.
